The Ministry of Health and Long-Term Care (MOHLTC)wants to understand how people find information on mental health, addictions, and gambling services. The MOHLTC hired Malatest, a national research firm, to complete this project. The following Frequently Asked Questions (FAQ) will give you information about the project, and how to complete the survey. Thank you in advance for sharing your experience.

Q           What is the purpose of this survey?
A           The purpose of this survey is to understand how you find (or would find) information about mental health, addictions, and gambling services.

 

Q           Why am I being contacted?
A           Your experiences are important to understand how people in Ontario get this information and help they need.

Please note: the MOHLTC approached MDAO and asked us to circulate this survey to our mailing list. MDAO is sending this survey to you directly, and has not shared your contact information with the Ministry.

 

Q           I don’t have a mental health and addictions issue and I don’t know anyone who does. Why am I being contacted?
A           If you have not experienced problems with mental health, addictions, or gambling and you do not know anyone who has, your opinions are still very important. We would like to understand where you would go in the future to get information/services, and your awareness of other services in the area of mental health, addictions, and problem gambling.

 

Q           Who is conducting the review?
A           The MOHLTC has hired Malatest, a national research form, to complete this survey.

 

Q           Will my survey answers affect the services myself or my family/friend(s) get?
A           No, what you say in your survey will not affect the services you and/or your family/friends get. The survey is being conducted by an independent firm (Malatest) to make sure that your responses are confidential.

 

Q           What will you do with the information?
A           Your survey will be summarized along with responses from other individuals who participate in the study. Your individual responses will be completely confidential and will not be shared with the MOHLTC or any other organizations.

 

Q           How long will the survey take to complete?
A           The survey will take no longer than 10 minutes to complete.

A           You may complete the survey before October 9th, 2015 by going online or calling Malatest:
